The Substance Misuse Team at HMP Nottigham, are seeking a dynamic enthusiastic Substance Misuse Practitioner to join the team. We are looking for someone who enjoys a challenge, can work well within a team and autonomously.

Key Responsibilities Include But Not Limited To

  • To carry a psychosocial case load with responsibility for assessing, care-planning, implementing and evaluating patient care.
  • To undertake comprehensive assessment of patients needs following referral to the service and completion formulations to advise patient care.
  • To plan programmes of psycho-social interventions using evidence based practice.
  • To lead/co-facilitate group work relevant to substance misuse and offending behaviour.
  • To effectively plan for patient discharge and prison transfer and/or release in conjunction with community services, ensuring appropriate transfer of patient information and management of highlighted patient risks including facilitating take-home naloxone training.
