Remuneration: Up to £14 per UDA
Description: Part-time Performer Dentist required for 4-5 days per week in a busy, mixed practice in Nottingham
• Up to 4500 UDAs
• Up to £14 per UDA depending on experience
• 50% split on Private & Labs
• Fully computerised surgeries
• Mon to Fri
• Established and stable patient NHS list & some private patients available
• Digital X-rays and Scanners
• Full support team
• Close to the town centre
• Available now, negotiable start date

Compliance requirements:- Full NHS Performer Number
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
