What You Will Do:

Conduct assessments for service users referred to the EIP Team.
Develop and implement recovery-focused care plans collaboratively with service users.
Deliver evidence-based interventions, including Family Interventions and CBT-informed care.
Administer medication safely, including depots when required.
Use outcome measures such as HoNOS, QPR, and DIALOG to monitor progress.
Provide clear information to service users, families, and carers regarding care plans, access, and confidentiality.
Build supportive relationships with service users and their families.
Utilize digital technologies to enhance service delivery.
Monitor and update risk management plans; ensure regular physical health checks.
Collaborate with General Practitioners, Consultant Psychiatrists, Inpatient Services, and other professionals.
Promote a culture of recovery, empowerment, and informed decision-making.
